Better Health Together Western Sydney Integrated Care program is for people who have chronic or long-term health conditions. It is a new way of providing support, offering care and services that keep you as well as possible in your home and local community. Your care will be planned with you by your healthcare team.

What’s in it for patients and families?

The Better Health Together Western Sydney Integrated Care program will allow you to have more care and support services.

A support person (called a Care Facilitator) will work with you to:

  • Help you to better understand what you can do to better manage your health condition yourself

  • Help you to monitor your health and know when to visit your GP

  • Help you get the support and health services you need to stay well at home

  • Plan your care with you and with your healthcare team.

Who can be part of the program?

The Better Health Together Western Sydney Integrated Care program, at the moment, is only open to patients with four specific chronic conditions:

  • Congestive cardiac failure
  • Coronary heart disease
  • Chronic obstructive pulmonary disease
  • Diabetes.

In the future, it will be expanded for patients with other chronic conditions.
